About the Role
Join Thames Reach as a Lead Manager within the Community Living and Support Service (CLaSS), where your understanding, compassion, and commitment will be valued in addressing the impact of street homelessness. Thames Reach delivers projects and services aimed at ending homelessness in and around London by working directly with homeless individuals on the streets and in hostels, preventing homelessness, and supporting formerly homeless people to rebuild their lives through work, training, and volunteering opportunities. Last year, we supported over 15,000 people and are recognised as one of the top 100 Best Workplaces in the UK.
The role involves line managing Lead Workers and co-managing the CLaSS Multi-Disciplinary Team (MDT), leading Multi-Disciplinary Meetings across the Trust, screening and allocating Adult Social Care referral cases, attending professional meetings and ward rounds to facilitate timely discharges, leading on Trust delayed discharges and Hospital Step Down processes, and liaising with clinical teams to ensure continuity of care in the community. This position is part of the Lambeth Living Well Network Alliance, which supports people experiencing mental illness in partnership with key local health and social care organisations.

Experience
- Proven ability to multitask and prioritise competing demands in a fast-paced, demanding environment.
- Experience in continual improvement of service delivery, ensuring high-quality, outcome-focused services involving internal and external stakeholders.
- Knowledge and experience working with vulnerable adults with severe needs.
- Understanding of barriers to hospital discharge and strategies to overcome them.

Qualifications
Relevant qualifications and experience in social care, health, or related fields are desirable. Willingness to undergo pre-employment screening including Disclosure and Barring Service checks is required. A criminal record will not automatically exclude applicants but must be disclosed as most posts are exempt from the Rehabilitation of Offenders Act 1974.
