About the Role
Join Thames Reach as a Lead Manager within the Community Living and Support Service (CLaSS). Thames Reach is dedicated to ending homelessness in and around London by working directly with people experiencing homelessness, preventing vulnerable individuals from becoming homeless, and supporting formerly homeless people to rebuild their lives. In this role, you will line manage Lead Workers and co-manage the CLaSS Multi-Disciplinary Team (MDT). You will lead Multi-Disciplinary Meetings across the Trust, screen and allocate Adult Social Care referral cases, and attend professional meetings and ward rounds to facilitate timely discharges. You will also lead on Trust delayed discharges and hospital step-downs, liaising closely with clinical teams to ensure continuity of care in the community.
The Lambeth Living Well Network Alliance supports people experiencing mental illness in Lambeth, and Thames Reach works in partnership with key local health and social care organisations to deliver innovative mental health services as part of this transformation.
Experience
- Proven ability to multitask and prioritise competing demands in a fast-paced, demanding environment.
- Experience in continual improvement of service delivery, ensuring high-quality, outcome-focused services involving internal and external stakeholders.
- Knowledge and experience working with vulnerable adults with severe and complex needs.
- Understanding of barriers to hospital discharge and strategies to overcome these.

Compassionate and committed to making a difference for people affected by homelessness and mental illness. You will be confident in leading teams and managing multi-disciplinary collaborations, with strong communication and organisational skills.
Qualifications
Relevant professional qualifications and experience in social care, health, or related fields are desirable. A commitment to safeguarding and willingness to undergo pre-employment screening is essential.
