About the Role
Join Thames Reach as a Lead Worker within the South East Prevention team, where your understanding, compassion, and commitment to supporting people affected by street homelessness will be highly valued. Thames Reach delivers projects and services aimed at ending homelessness in and around London by working directly with people on the streets, in hostels, and by preventing vulnerable individuals from becoming homeless. Our prevention services provide skills, housing advice, accommodation, and tenancy sustainment support to help people transition towards independent living.
As a Lead Worker, you will provide a rapid response to referrals of people presenting as homeless across support services in the South East London region. You will use risk assessment tools to determine the likelihood of rough sleeping and deliver appropriate interventions. Your role will involve providing pathways ranging from signposting to community services to short-term casework or accommodation outcomes. You will collaborate closely with partner agencies, including day centres and local authorities, to ensure clients access the support they need to secure accommodation. Additionally, you will represent the team at multi-agency meetings and maintain accurate records of all work completed within required timeframes.

Experience
- Proven experience working with vulnerable adults and supporting them to access services.
- Ability to manage a high caseload efficiently with excellent organisation and time management skills.
- Experience in using IT systems including Outlook, online recording systems, Excel, and Word, as well as tablets and smartphones.

- Ability to build and maintain strong working relationships with partner agencies, including the confidence to challenge when necessary.
